GULF COAST JEWISH FAMILY:

• PROGRAM: The mission of the Heart Gallery of Pinellas and Pasco County is to connect children eligible for adoption in the Florida Foster Care System with their forever families. We focus on the “hardest to adopt” children: sibling groups, teenagers, and children with advanced medical needs. We do not stop recruiting until a child is adopted, turns 18, or their legal status changes. The Heart Gallery also supports children and adoptive families before, during, and after adoption to ensure their successful adoption journey. Heart Gallery promotes local foster child adoptions through Heart Art, portraits, and recruitment tools, fostering emotional connections and celebrating successful adoptions in Pinellas and Pasco Counties. Showcasing that Pinellas and Pasco (Circuit 6) have the most children entering foster care in the country.

• QUALIFICATIONS FOR PARTICIPATION : A family will need to have an approved home study. When a family does not have an approved home study, they are referred to their local case management organization to start the process. We provide the following statement when we receive an inquiry without an approved home study. The first step on your journey is to attend an official fostering and adopting orientation provided by your local Child Welfare Lead Agency. At the orientation, you will be provided information and instruction on how to take the next step in becoming either a foster parent or adoptive parent.

PASCO KIDS FIRST :

• PROGRAM: Healthy Families Pasco Hernando: An abuse prevention and home visiting program. Family Support Specialist (FSS) conduct home visits on a weekly basis, provide in home curriculum-based parent trainings from the “Growing Great Kids/ Growing Great Families & Fatherhood in 15” program, offer ongoing father engagement and monthly family support outreach activities. The program serves hundreds of families every year; 458 families in 2022 and conducted 3,596 home visits. The Healthy Families program consists of highly trained and specialized professionals who provide unduplicated services to residents of our Pasco community.

BAY AREA LEGAL SERVICES:

• PROGRAM : Legal Support from Human Trafficking Program provides holistic legal support to survivors, offering services such as restraining orders, divorce assistance, child and spousal support, custody and visitation orders. Emphasizes empowerment and safety for survivors. We provide high-quality, free legal services to vulnerable populations, including seniors, low-income individuals, victims of domestic violence and human trafficking, and veterans. Clients with limited access to legal services are assisted with advice, referral, limited or full representation in areas of civil litigation. Legal services are provided in cases involving family law, domestic violence, immigration, housing and landlord-tenant, public benefits, disaster relief, consumer, tax, health, and other legal matters. Bay Area Legal Services, Inc. strives to create pathways to justice for the underserved.

• QUALIFICATION TO PARTICIPATE : As a grantee of the Legal Services Corporation, Bay Area Legal Services can only provide legal assistance to a U.S. citizen or eligible undocumented persons and permanent residents in accordance with the Legal Services Act. Otherwise, those determined as ineligible for legal assistance and are given referrals to other agencies or services within the local community. BRIDGING FREEDOM :

• PROGRAM : Bridging Freedom’s rehabilitation program is located in a safe and secure campus with several homes and space for outdoor therapeutic activities. Children live in a home with their own bedrooms and bathrooms, and nurturing residential caretakers who create a positive, healthy and stable environment conducive to healing. Survivors receive: Individual therapeutic sessions from clinical therapists; Health care management from a registered nurse; Psychoeducation groups, such as Substance abuse, Anger Management, Grief and Loss, Healthy Boundaries, etc.; Therapeutic activities, such as equine art and gardening; Online education through FLVS; Monthly educational field trips and weekly fun recreational outings; Life skills to help survivors adjust to life outside of the home; Transitional mentorship to provide support for 18 year old survivors after they graduate the program. Through a step-wise program that last approximately a year to 18 months, we help children begin to stabilize, trust again, heal emotionally and physically and adjust to being kids again.

CHILDREN’S HOME NETWORK:

• PROGRAM: The CHANCE program is an STFC program for victims of Commercial Sexual Exploitation. The program was developed by Citrus Health Network in 2013, and CHN was privileged to be able to partner with them in order to expand the program to Pinellas/Pasco Counties. CHANCE Safe Home families receive the same 30hr pre-service training as STFC parents, with an additional 24hrs of training specifically on Human Trafficking. CHANCE Safe Homes only serve 1 child at a time, and families receive complete wraparound support including psychiatric care through a contracted provider, a therapist who visits 2-5 times per week, a targeted case manager to assist with coordination of care, and a life coach mentor with lived experience as a survivor of human trafficking to support the youth.

I Rise

• PROGRAM : Trauma Informed Workforce Development Program, provides 120 hours of financial literacy, psychoeducation, soft skills and hard skills training over the course of four weeks to equip survivors of violence and exploitation with the skills and readiness needed to be successful in the workplace. I Rise focuses on helping survivors discover a career path and works to help women get connected to education/programs and employment along that career path. The ultimate goal of the trauma informed workforce development program is to break the cycle of poverty that pulls women back into a violent or exploitative situation, and to support financial independence, stability and upward mobility for survivors.

SELAH FREEDOM :

• PROGRAM: Freedom for Youth Prevention Training : Selah Freedom is a faith-based nonprofit anti-human trafficking organization, based in Florida and the Midwest, with the mission to end sex trafficking and bring freedom to the exploited through five strong programs: Awareness, Prevention, Outreach, Residential and Organizational Consulting. The Freedom for Youth Prevention Training program educates students K-12 on the dangers of human trafficking through innovative training and age-appropriate curriculum. Our Prevention Team provides trainings at local schools and community organizations and offers virtual trainings. Youth are taught how to identify and report unsafe situations and are equipped with the knowledge to stay safe.

• PROGRAM: Outreach and Shelter: provides comprehensive domestic and sexual violence services, including a 24-hour hotline, an empowerment-focused intervention team, prevention initiatives, and a confidential emergency shelter for survivors with various support services. Sunrise provides emergency shelter for survivors of domestic and sexual violence, to include; counseling, safety planning, case management, food, transportation, legal advocacy. Additionally, Sunrise provides outreach services to survivors of domestic and sexual violence to include; counseling, advocacy, legal advocacy and representation, forensic exam accompaniment, information/referral and safety planning.

THE ARC NATURE COAST:

• PROGRAMS: Community Support for Individuals with Intellectual and Developmental Disabilities and Collaborative Disability Employment Initiative: Individuals with disabilities often require professional support to secure employment. Pre-employment job development is crucial for their success, especially for those facing challenges in reading, driving, or navigating the employment process. While they may struggle with certain tasks, they can excel in job duties. To address unemployment among people with disabilities, our assistance includes transportation, completing applications, interview support, job coaching, and troubleshooting any issues that may arise. We firmly believe that employing individuals with disabilities not only strengthens businesses and communities but also contributes to making the world a better place.

DEAF AND HARD OF HEARING:

• PROGRAM : Signing for Everyday Life: Offers educational training and workshops to include sign language classes, computer classes, sensitivity training, and various seminars to help the hearing-impaired community. Community education includes the Signing For Everyday Living Program, which teaches how to live with hearing loss ; Communication Access Assistance: Provides services to individuals who are experiencing a hearing impairment and need assistance in accessing the available services within the community. The Organization has recently added mental health services to the CAA program.

HABITAT FOR HUMANITY- WEST PASCO AND PINELLAS :

• PROGRAM: Economic mobility through Affordable Housing , he Habitat for Humanity of Pinellas and West Pasco Counties program aims to increase economic mobility through affordable homeownership, targeting Asset Limited Income Constrained Employed (ALICE) households in West Pasco County. Economic mobility is a hallmark of any healthy society and one of the primary avenues American's build wealth in our society is through homeownership. However, affordable housing continues to be a crisis throughout our region and more specifically access to affordable homeownership, further exacerbating income inequality. The Habitat Homeownership Program offers local families and individuals a hand up towards safety and stability through affordable homeownership.

• QUALIFICATION FOR PARTICIPATION: Participants need to meet certain eligibility requirements to participate in the program. All participants are prescreened prior to program entrance to ensure eligibility. Eligibility into the program will be income limited, serving low to moderate income individuals/families between 30%-80% of the area median income. Additionally, participants will need to have an actual housing need. This can mean yet is not limited to living in overcrowded conditions, paying more than 30% of income in rent, residing in substandard housing or unsafe conditions. Lastly, other minimum requirements include having lived or worked in Pasco County for the past year, proof of one year of reliable income, demonstrate an ability to make regular payments on their bills and ability to afford a mortgage.

• PROGRAM: Habitat works toward our vision in partnership with families in need of decent and affordable housing. Habitat homeowners help build their own homes alongside volunteers and pay an affordable mortgage.

ONE COMMUNITY NOW:

• The One Community Now Resource Center focuses on elevating families out of poverty by providing various services, including financial assistance, community resource sign-up, state ID/birth certificates, and job-related support. They also offer homelessness prevention for income-qualified individuals and empower employment success through comprehensive job-seeking resources.

• Resource Center - We assist individuals and families that are struggling financially by providing employment resources including resume creation, interview coaching, job search and basic resources. At various times we have funding to assist with rent, utilities, housing and employment.

• Pack A Sack Food 4 Kids - We provide food for children that struggle with food-insecurity. Children are provided with a bag full of food for each weekend of the year.

• Veterans Support Center - We focus on helping veterans get connected to the resources they need to build stability in their life. We provide resources that include food, hygiene items, housing assistance, showers, laundry services, employment, housing, physical and mental health.

YMCA-TAMPA METROPOLITAN

• PROGRAM: Veggie Van: a Mobile Marketplace: We address food deserts in Hillsborough and Pasco counties, offering fresh produce, nutritional guidance, and health support to combat food insecurity, all supported by various organizations. Donations are encouraged. We go to food swamps across Hillsborough and East Pasco County. In Pasco County, we serve the community around Rodney B. Cox Elementary School. Services are accessed at Rodney B. Cox Elementary School in Dade City at school pickup. No qualifying requirements or fees are required to participate and clients do not need to show any proof of income level. Each family receives one box of produce at the biweekly stop (approximately 15 pounds each). The boxes typically include one leafy green, three other vegetables, three fruits, and tomatoes.

CHAPTERS HEALTH FOUNDATION:

• PROGRAM: Charity Hospice and Respite Care are offered in Pasco County by HPH Hospice to any person in need of end-of-life care, regardless of age, race, gender, religious beliefs, or cultural background, and who is unable to pay privately or through insurance coverage or other means. We enable everyone under their care to die with dignity, and we provide dying patients with the highest quality of life possible for as long as possible. HPH has facilitated and witnessed homeless and dying people reunited with long-lost family members. Even when without family, our patients do not die alone on the streets or in rundown places. They are cared for, clean, well-fed, comforted, and treated with respect, sometimes for the first time in years. Whether outpatient at home, inpatient hospice care, or wherever the person calls "home," HPH Hospice staff help the people under their care to understand the nature of their illness or disease fully, understand their options for care, and develop a care plan that aligns with their personal beliefs and desires.

• QUALIFICATION FOR PARTICIPATION : a chronically ill person-with or without shelter would possess no reliable source of income or health insurance or be defined as an ALICE patient. They may align with the Medicaid eligibility guidelines. The patient would need to be diagnosed and referred by a physician, and all patients are assessed by clinical staff and social workers.

CHILDREN’S HOME NETWORK:

• PROGRAM: Caregiver Health & Wellness Initiative focuses on promoting self-care for relative and non-relative caregivers within Pasco County. This service provides access to group meetings educates them on the topics of self-care, and stress management, and provides community resources such as benefits navigation and referrals to other agencies. CHN partners with community agencies to provide additional concrete support and services to families. Caregivers enroll in our Kinship program, which provides navigation services and offers our Time for Me self-care services. These sessions invite guest speakers and subject matter specialists such as nurse practitioners, grief/coping counselors, nutritionists, and physical trainers to attend and discuss better self-care practices and other available services. These group opportunities offer educational information that promotes a small-change approach in our caregivers' families.

FEEDING PASCO’S ELDERLY:

• PROGRAM: Feeding the Hidden Hungry, Since 2014, Feeding Pasco’s Elderly, an all-volunteer-led nonprofit organization, has served homebound senior adults in Pasco County. We work collaboratively with community agencies, business leaders, government officials, and citizens to raise funds, engage the community, and increase home-delivered meal services through the infrastructure of the Pasco County Senior Services Division.

• QUALIFICATION FOR PARTICIPATION : Seniors in Pasco County who are food insecure and have been placed on the wait list for meals by the Pasco County Senior Services Division.

FRESH START FOR PASCO:

• PROGRAM: People Helping People program, This program targets low-income families, Elderly and veterans. We help with rents, deposits and utilities and food. This is an income-based help must be under the 80% median income. Because of the grant amount we may be only to help with a portion of the clients financial need, but we will work with partner agencies to help with additional funding.

• QUALIFICAITON FOR PARTICIPATION : All applicants are given an application and interviewed by a case worker to determine the clients need. Proof of residency and income is required. Clients can reach us through the website, Facebook page or call. Walkins are welcome. This is an income-based criteria again must be under the 80% median income. Clients must be a resident of Pasco County.

• PROGRAM: Project Thrive: We offer a food pantry at our main facility, where clients can pick up food access our services. Our food pantry is available Monday-Friday from 8:30am till noon. And we can include meals for their pets. The client is able to come twice a month. Our Brown Bag program focuses on low-income, disabled, and homebound seniors who are physically unable to get food. BAYCARE BEHAVIORAL HEALTH:

• PROGRAM: Medical Outpatient Services:, This service offers education, socialization, employment skills, and support to individuals who have a mental healthdiagnosis. It is a peer lead, peer support program with staff support that helps to empower all participants to move them towards recovery. The primary goal is for an individual to become more productive in the community and eliminate or reduce the need for inpatient crisis stabilization. We reduce relapse rates as well as reduce costly inpatient behavioral health hospitalizations. We want the client to remain engaged in treatment and adhere to treatment plan goals or be successfully discharged from medical services. Medical management services are provided by Psychiatrists and APRNs, with support of nurses and psychiatric techs. We consider the medical history and all medications that are currently being prescribed for contraindications. We grow a relationship with patient’s PCP and our medicaloutpatient team. When prescribing medication, the medical staff considers many factors in addition to the individual's overall health. An example of this is the consideration of the individual's payer source and ability to pay for and/or obtain the prescribed medication. • PROGRAM: Pasco County Alliance for Substance Addiction Prevention (ASAP) aims to improve the health in our communities through cross-sector partnerships and programs. • PROGRAM: We provide our evidence-based, one-to-one mentoring programsfor youth who are at risk for academic failure or involvement with the juvenile justice system. Community Based Mentoring where matches meet in the community 2-3 times a month for an average of 8 hours, School Based Mentoring where matches meet at the child’s school once a week for one hour, and School-To-Work Mentoring which serves high school juniors and matches them with a workplace mentor for two years and who meet half a day a month during their junior and senior years. BOYS AND GIRLS CLUB OF TAMPA BAY

• PROGRAMS: Power Hour brings high-level tutoring and learning activities that stimulate the academic growth of Chasco Middle School students and Lewis Abraham Lacoochee Boys and Girls Club members. Over 80%-90% respectively qualify for free or reduced lunch. Youth receive homework help, tutoring, and other high-yield learning activities led by youth development professionals and volunteers. The Chasco Middle School Summer Reading Program helps students improve their Florida Standards Assessments (FSA) scores in Math and English Language Arts through fun instruction and activities. We utilize the assistance of both on-site teachers and online Assessment and Learning in Knowledge Spaces (ALEKS) and Study Island programs to provide high quality, on-grade learning activities and tutoring sessions. These are are digital learning systems with interactive features, questions, and games that engage and reinforce student learning.

SUNCOAST VOICES FOR CHILDREN:

• PROGRAMS: While it is sometimes necessary for safety reasons for the CPI Unit to remove children from parents, the disruption to the children's lives caused by the removal is immeasurable. Along with being removed from their immediate families, children entering foster care typically face separation from their schools, their teachers, their neighborhood, their friends, their extracurricular activities, and their doctors and therapists. With the Pasco Safe With Family Project, Voices provides beds, car seats and other safety equipment immediately so that children can be placed with safe, loving grandparents, aunts, and uncles.

• Styled Voice Project: operates five locations providing immediate access to free clothing, toiletries, and supplies for children entering foster care. Voices provide four outfits, underwear, socks, shoes, and personal hygiene items to children entering foster care at no cost.
